Cloud Readiness Assessment Questionnaire

Last updated on

This questionnaire builds the baseline for the STACKIT Cloud Readiness Assessment of your workloads. The gathered data is mandatory for a successful workshop series and detailed migration planning.

Please answer all questions as precisely as possible and return the completed document to your STACKIT workshop lead so the team can prepare the sessions according to your specific environment.

  1. Document Creation: Copy the questionnaire section into your internal documentation template.
  2. Data Collection: Complete all sections with your technical stakeholders. Detailed information provides the best foundation for workload analysis.
  3. Submission & Review: Send the completed questionnaire to your STACKIT contact person prior to the expert workshop phase.

  • Company Name: Name of the organization.
  • Address: Primary office address.
  • Primary Contact Person: Main technical or project contact.
  • Secondary Contact Person: Alternative contact person.
  • Total Systems Count: Total number of IT systems in scope.
  • Current Cloud Providers: Active hyperscalers or private cloud providers.
  • Target Workloads to Migrate: Estimated number of workloads targeted for migration.
  • Schwarz Digits Support: Indication whether dedicated support from Schwarz Digits is requested.
  • Hypervisor Details: Current hypervisor platforms and active software versions.
  • Physical Footprint: Total number of physical servers.
  • Virtual Footprint: Total number of virtual machines (VMs).
  • Compute Sizing: Allocated vCPU cores and total RAM in GB/TB.
  • Resource Utilization: Median capacity usage percentage across compute and memory.
  • Storage Infrastructure: In-use storage types (block, file, object, SAN/NAS).
  • Storage Performance: Median required IOPS and throughput metrics.
  • Peak Performance: Maximum required IOPS and throughput during peak loads.
  • Disaster Recovery & SLAs: Current backup strategy, failover testing frequency, RTO, and RPO SLAs.
  • Constraints & Licensing: Hardware or licensing constraints (BYOL, dedicated hosts, unsupported OS).
  • Database & Middleware: Managed databases or middleware platforms currently deployed.
  • Container Infrastructure: Status of containerized workloads and orchestration engines (e.g., SKE/Kubernetes).
  • Container Roadmap: Future plans for containerization if not yet deployed.
  • Serverless Architectures: Usage of serverless or event-driven execution models.
  • Lifecycle Management: Patch management, updates, and lifecycle processes currently enforced.
  • Architecture Landscape: Structural composition (monolithic vs. microservices, commercial off-the-shelf vs. custom solutions).
  • Dependency Mapping: Availability and sharing permissions for application dependency mapping.
  • Service Level Agreements: Active availability, recovery, and performance SLAs.
  • Technical Debt: Known architectural, security, or platform technical debt.
  • Migration Scope: Preferred initial workload candidates for cloud migration.
